Short answers, you print it, GM fills it out and turns it in to you, you make sure it’s complete before submitting to Payouts.

Typically each department handles their own paperwork including the payout part of the forms. Someone supervising the department on site is ultimately in charge of the paperwork and making sure the GM fills it out properly. That includes reminding GMs to get player data before play starts.
