Yes, well right now for RPG/RPGA/LARP most of it should be done right before the game starts. Only documenting winners, if there are any, would be after the game. Also, any questions that get listed can be answered at any appropriate time before turning in the sheet.

Probably, though since I don't have copies of the sheets turned into payouts, I can't say yes.

Ignore the term slot. I will remove it. I was trying to label each column. It's a signup sheet. Since RPGs have limited space, each space is accounted for. And since RPG/RPGA/LARPs rarely have actual payouts, it's an all inclusive sheet.
One sheet per game run, if you have multiple offerings of the same game, each gets a sheet.

Initial goal is to be able to track participants. What they show interest in (signup sheet) and what they actually attend.

That would be because no one in has given me any questions or suggestions on what to add (to these sheets). These have to be questions GMs can answer without polling the participants. [The questionaire/survey will become a new discussion]
Boardgames has # of rounds, # of games, time for each round and "Where there enough copies of the game for the first round?", and "How many people needed to be taught the game before playing?"

I'm not a role player, so my knowledge base is limited. The only question that I'm guessing may have bearing on RPG and/or RPGA is the last one, but I don't know. What else might be relevant, I don't know. What differences there are between RPG and RPGA, I'm not really sure. If it's relevant to data collection, enlighten me, please.
