Might want to add a phone number as an optional field.

Might want to remove the departments that weren’t active for Gamex, like Collectibles and Family Games for the first list.
Why is Artemis not on the first list?

The tabletop simulators question should either have two lists, Heard of, and Recommend or be two separate questions.

How about other simulators not listed?

Consider adding would you be willing to pay extra fees to access video game or collectible events.
