A shot at a free badge is better than a small discount and Janice is
right - you can expect better numberswith online surveys. My company
uses Zoomerang all the time and when we incent, we get more than 30%.
When we don't, it is about half that. Keeping it short, mostly radio
button (with one place for comments) helps. Just send it out with a
positive message that we're listening and want their ideas. It will be
good. Plus, it"s cheap (just the cost of the badge giveaways - and
five is plenty).
